  1. Why was Community Collaboration Inc. established?
    CCI has been established to provide intuitive tools and methods that enable collaboration among all stakeholders in the shuman service sector, empowering them to take a transformative approach toward the delivery of services.

  2. What is CCI’s purpose?
    The core purpose of the organization is support community leaders, government officials, program funders and human service providers that continually seek innovative human service delivery practices that reduce lifecycle costs, save public tax dollars, break cycles of dependency, and create wealth within the community.

  3. How does CCI’s case management system decrease client dependency on the social service system?
    CCI has developed best practices and effective processes for case workers to use in effectively establishing goals in 14 areas of life. The result is a comprehensive and holistic plan designed to promote self-sufficiency. Through coordinated services, case managers are able to focus on their specific service area which provides the client with clear guidance and comprehensive services toward the empowerment of goal achievement. Our solutions enable human service providers to share a collaborative platform which provides the medium for service providers to create a single plan for mutual clients. Each provider can focus on their service area to create goals while the coordinated planning offers the client a consistent planning method throughout all areas of service. The providers can then work together by providing progress reports, notes, and discussion on guiding their client toward self sufficiency.

  4. What if an organization prefers to operate independently in a community? 
    There are many reasons some organizations may prefer to operate independently: privacy and security; ineffective agency relations; fear of losing funding resulting in having to cut services and possibly staff members. We perceive independent services and competition for program funding as barriers to effective practices as well as opportunities to obtain program funding that would help grow the organization.

    CCI has been a thought leader in the national movement for collaborative practices and operations. This movement is rapidly expanding and the organizations that remain independent from the collaborative approach, could suffer from the very fears that keep them working as an independent agency. Through transforming from an independent operator to a collaborative member of your community, your services, resources, and efficiencies will increase with purpose and promise.

  5. What measures are in place to ensure privacy, confidentiality and security?
    The system is hosted on servers at Northern Illinois University (a secure national data center) that meets stringent data security requirements. The CCI collaborative platform is compliant with federal and Illinois State regulations governing security, privacy and confidentiality such as: Confidentiality of Alcohol and Drug Abuse Patient Records; Health Insurance Portability and Accountability Act (HIPAA); Standards for Privacy of Individually Identifiable Health Information (the “Privacy Rule”); Security Standards for the Protection of Electronic Protected Health Information (the “Security Rule”); and HUD Homeless Management Information System (HMIS) requirements. Compliance is proactively monitored and new regulations are reviewed and included as required.

    Clearly the privacy and security of personally identifiable information is key to making this solution viable and sustainable. A “Community Database System Privacy White Paper” has been produced to highlight key points and current legal research. The CCI system has been found to meet or exceed the high standards for protecting personally identifiable information which have been established under applicable laws and the policies and procedures of all the system’s licensees.

  6. How will collaborating with other agencies affect competition for funding between agencies?
    CCI is a thought leader in the movement of collaborative human service delivery practices and community wide improvement plans. New opportunities in grant funding are in the form of collaborative services and community initiatives. By putting the tools and processes in place to bring about a network of services for the citizens in your community, new funding opportunities will open up, inside and outside of your traditional funding programs. Your community and human service delivery sector will become a collaborative network of resources working together on plans and services to lift citizens out of dependency and into productive lives.

  7. How will you measure whether this initiative is successful?
    A forum of community leaders will set goals for the community and agree on measures that will indicate success in achieving those goals. Examples of goals and measurements might include jail census and recidivism, truancy rates, recovery from homelessness or unemployment and crime rates. Coalitions of community resources will work together to improve collaborative interventions in an effort to meet these goals. Reports on measures will indicate success and guide future interventions.

  8. What return on investment can be expected from this initiative?
    There is no doubt that strategic implementation of CCI’s collaborative case management platform along with establishing a community-wide framework and methodology for collaboration amongst service providers will provide substantial financial benefits to communities at large.

    Conservative estimates of savings in select community initiatives in the areas of truancy, reduced recidivism, placement in stabilized housing as well as modest improvements in organizational efficiency and productivity reflect a return in excess of $11,000,000 in one local community over a three-year period. Any one of these examples justifies the system and entity investment many times over. Collectively, they represent a potentially revolutionary impact on the community and in the lives of those for which we seek to provide assistance.
