Amicus is a Minnesota not-for-profit organization with over 44 years of experience in improving public safety by helping inmates and ex-offenders through positive relationship-building, restorative justice practices, and individualized transition services.
When Amicus was awarded the Community Funding Grant from Wells Fargo at the end of last year, the organization selected CCI's MPOWR system to enhance their data collection, case planning, and reporting systems for their programs. Beginning with their Radius program, Amicus will be assisting girls emerging from the juvenile system to choose positive paths in their lives. Amicus then plans on rolling out MPOWR to the remainder of it's programs. This will enable broader collaboration with the other reentry and recovery agencies already using these tools across the Twin Cities.
We look foward to partnering with Amicus in it's mission to help ex-offenders and juvenile offenders to build successful lives and stronger communities.
CCI is excited to announce their recent application for the Health Care Innovation Challenge (HCIC) Grant. CCI was introduced in the spring of 2010 to a developing consortium of medical professionals in Wisconsin that were interested in combining the traditional medical model with the behavioral or 'social determinants of health'. They were excited that CCI had a comprehensive system to assess, plan, and monitor behavioral components in multiple core areas of life through a HIPAA compliant web-based platform.
Over the last several months, they introduced CCI to several additional partners in both the medical and behavioral arenas, and the developing relationships formed an ideal foundation for the Health Care Innovation Challenge grant. It was decided that CCI, along with its Rockford based ally SupplyCore, would be the preferred applicant with our combined experience in collaborative software development and data integration. The grant will be a dual state initiative with implementations and partners in Wisconsin and Rockford.
The HCIC is a one billion dollar national grant funded through Health and Human Services (HHS). Grant recipients will receive their awards over three years, ranging from $1,000,000 to $10,000,000 per year. CCI applied for more than $16,000,000 over the three year grant period.
If the application is successful, advantages for Rockford will include: 1) increased health and welfare of the uninsured and underinsured in our community; 2) a significant portion of the grant application being administered and spent locally in the form of new jobs including a new breed of workforce development with cross training in both medical and behavioral aspects; 3) expansion of our local collaborative efforts; 4) significant positive recognition for Rockford.
Our commitment to Rockford and building a better Rockford has remained strong as we experience tremendous growth on a national scale, and continue to search for opportunities to bring additional jobs, revenue streams, and positive recognition into Rockford.
Bridges Out Of Poverty has recently adopted the CCI model for holistic and collaborative care to assist individuals and families in removing themselves from poverty. Bridges will soon be implementing a national marketing campaign to implement CCI throughout their national network of over 50 communities in 20 states.
CCI supports Move The Mountain 'Circles' national anti-poverty initiatives that were featured recently on CBS national news for their positive impact on reducing poverty. Circles is active in several states and utilizes a CCI customized application to fit their specific additional data and reporting needs. Circles recently renewed its contract with CCI and is initiating an increased effort to implement CCI on a broader basis throughout their multi-state networks.
CCI's Continuums of Care currently operate in 24 States plus One Canadian Providence. We are honored to assist over 100 active agencies, which employ over 1,100 users/case managers, who assist approximately a quarter million unduplicated individuals to overcome long-term barriers and achieve their life goals.
Community Collaboration and Integration is proud to announce that the Rockford Housing Authority has selected CCI's MPOWR Case Management System to aid RHA case managers and residents in a new emphasis on case management to plan for success, and achieve residents personal goals toward increased economic self-sufficiency.
RHA has always been a respected provider of safe, decent, and sanitary housing. They are now initiating a renewed vision to become a housing provider that meets an even higher level of individual community success. In partnership with CCI and other local agencies, RHA is not only serving people IN their distressed condition, but is working holistically and collaboratively to lift people OUT of their distressed condition. This initiative has the goal of empowering residents to gain economic self-sufficiency that will enable residents to 'move in, move up, and move on'.
CCI has imported over 2,800 individuals from RHA's existing system into the MPOWR system to serve the individuals and families in all of their Self-Sufficiency programs now known as ReBuild -- Your Life, Your Choice.
CCI is the developer of the data tools used for the Northside Achievement Zone (NAZ), enabling families to develop a "cradle to college and career" pipeline, locally branded under the name "NAZ Connect." CCI has had the privilege to be a part of the growth of NAZ over the past 4 years and now celebrates with Minneapolis on the Promise Neighborhood award.
