Sophisticated Tools for Collaboration

Collaborate. Innovate. Accelerate.

CCI allows agencies to collaborate so that all appropriate resources can be engaged to facilitate successful implementation of each individual’s unique plan. The solution enables agencies to work together to:

  • Set goals
  • Monitor progress
  • Record accomplishments
  • Adjust the plan as needed

With appropriate client authorization, CCI makes it easy for all agencies working with a client to share comprehensive client information – from the individual’s unique plan and case notes to utilized services, resources, and outcomes.

The solution’s database provides extensive capabilities to support effective collaboration:

  • Records and sorts case notes
  • Records resources provided
  • Issues alerts
  • Identifies restrictions
  • Provides notifications of appointments
  • Records messages for clients
  • Includes a comprehensive and searchable Providers Database to easily locate required resources

Security, Privacy and Confidentiality

The CCI system is hosted in a secure national data center and meets stringent data security requirements. The application is compliant with federal and state regulations governing security, privacy and confidentiality, such as:

  • Health Insurance Portability and Accountability Act (HIPAA)
  • Standards for Privacy of Individually Identifiable Health Information (the “Privacy Rule”)
  • Security Standards for the Protection of Electronic Protected Health Information (the “Security Rule”)
  • HUD Homeless Management Information System (HMIS) requirements

Compliance is proactively monitored and regulations are systematically reviewed and included as required.

  • CCI’s proprietary four-level data sharing privacy and confidentiality model protects sensitive client information, and is uniquely friendly for both clients and agency staff
  • Sharing is triggered from a system-generated, multi-level client release of information
  • Information sharing is time bounded and may be revoked by the client at any time
  • Any information may be designated as private and not shared, even with a signed release
  • All community outcome data is aggregate-only, with no client identifiers
  • All data sharing protocols and technology have been reviewed by legal specialists in intellectual property, privacy, computer, and internet-related issues

“The implementation of CCI services has improved the effectiveness of client services. The ability to gain a thorough perspective in all areas of life and collaborate with partner agencies through shared technology has given us the ability to connect our students to specific service-oriented providers in a speedy fashion.
